Primary care visits are included in the monthly fee. No deductible threshold and no annual limit. Mental health is included at 12 visits per year. Dental includes 2 cleanings and basic care annually. Vision includes an annual exam and prescription correction. You go when you need to.
Preventive care
Annual physicals, bloodwork, screenings — no billing.
All preventive care — annual physicals, bloodwork, cancer screenings, immunizations — is covered with no separate billing event. The DPC model makes prevention economically rational for the physician: a steward who never gets sick costs the clinic nothing. The incentives are aligned.
Mental health
12 visits per year. Included.
Mental health visits are included at 12 per year, with no separate deductible and no network requirement. Your therapist or counselor is a steward resident — the same person you'll see at the Spice Tray café. The stigma of cost that keeps people from going is gone.
Specialist referral
Referred inside the fee. No bill reaches you.
A referral to a specialist is handled the same way as everything else clinical: the town negotiates the contract and settles the billing directly. Nothing about the referral generates an invoice for you, and there is no claim to file, no deductible to clear and no network to check first. Your physician at the Blue Tulip clinic decides you need a specialist, and that is the end of your involvement in the money side of it.
Out-of-town ER
Negotiated and paid by the town.
If you need emergency care outside the town — an accident while traveling, a sudden event away from home — the town's emergency care escrow negotiates and pays the bill directly. No surprise invoice arrives months later. The out-of-network line funds it.
Open-source pharma
Drugs at manufacturing cost across all URBAL towns.
Generic drug production, federated across the URBAL network and delivered at manufacturing cost — eliminating the markup that makes prescription management a financial burden for most households. Your pharmacy is built into the Blue Tulip clinic alongside OSS Pharma. No errand. The drug you need costs what it costs to make.
Open-source hardware

Clinical equipment and implantable devices are designed open-source across the URBAL network rather than licensed from a manufacturer at whatever the market will bear. It is a real line in your monthly fee, alongside the drugs — together they are the only part of a health system anywhere that funds its own research and hands the result to everyone.
